Before you ship:

1. Fill out and send this form so we can look out for your package. You can be as specific in the Message section as you like.

2. Include a return shipping label with your package. ***Insure your package for replacement cost.***

3. Leave a love note for us in your box with: your name, phone number, email address, and return shipping address.

   
   ***Items received without identifying information may cause delays to your repair.
   

4. When we receive your equipment, we will send you a customer portal set-up email and an invoice for your bench and/or express fee.
   
   Bench and/or express payments must be received before we can begin work on your equipment.
